After you fill out the Book an Appointment form we’ll contact you to find a good time for your appointment on the date you selected. If you can’t make your scheduled appointment please let us know at least 24 hours in advance.
If you are experiencing a medical emergency please call 911 immediately. For anything less than an emergency, full out the form to Book an Appointment, or call 206.632.1337 to make an appointment.
